Request Survey Responses via QR Code

Allow respondents to weigh in without collecting contact information

Patient Experience Insights surveys support QR codes and static links for soliciting responses in situations where you may not have contact information. Respondents can scan the code from a mobile device (or type in the link)

Setup

You can enable this for any survey by visiting Patient Experience and selecting the three-dot More menu for your survey of choice, then Generate QR Code. Swell will create a unique link and QR code for your survey, which you can download and print on e.g. posters and business cards. You can also continue to send the survey to contacts and via automation after generating a QR code.

Limitations

It's important to note that responses gathered via static link or QR code have several limitations.:

  • They contain no information about the respondent, so unless you include a question for them to provide their name or contact information, their responses will be anonymous.
  • They lack appointment metadata and are therefore excluded from automatic report filtering like By Location, By Provider (NPS), or By Procedure Code (NPS).
  • Responses gathered via QR Code are grouped under a "No location" category for filtering purposes.
  • Swell uses completion tracking to attempt to prevent multiple submissions from the same person or device, but this cannot be fully guaranteed due to technical limitations.

For these reasons, we recommend using QR codes for situations where detailed information about the respondent is not required.
